POST-ACCEPTANCE PAPER FORMS FOR STUDENTS GOING TO FRANCE
Congratulations on being accepted to one of ASA's programs in France!
When are my forms due?
Once you've been accepted, please remember that your post-acceptance forms are due by the application deadline or by 2 weeks after the day we sent you your acceptance email, whichever comes first! Exceptions to this deadline are 1) Flight Itinerary Form - due 6 weeks prior to departure;
2) Proof of Physical Examination - due 2 weeks prior to departure. Students using
financial aid to pay for the program - your 2 Financial Aid Certification Forms are due
by the application/payment deadline.
Late or missing paperwork will result in diminished selection of courses and housing.
Please don't forget that we can't enroll you in your desired program without your
completed forms!
Accepted students should also be aware that the application deadline is also the full payment deadline.

HOW TO COMPLETE YOUR PAPER FORMS:
All paper forms can be found below. You need to PRINT OUT each required form, complete it and return it to ASA by fax
to 617-327-9390. You can also mail them in, or email them to chelsea@academicstudies.com as a PDF (no .jpgs!)
I. PROGRAM CONTRACT - Download the Program Contract for your city and term. Print the form, read, sign and return to ASA.

II. COURSE PRE-REGISTRATION FORM - Download the Course Pre-reg form for your program/term. Print the form, read, sign and return to ASA.

III. OFFER LETTER - This will be mailed to you with your acceptance packet. You'll need to
read, sign and return this letter to ASA along with your $750 Housing Deposit.

V. FORMS FOR STUDENTS USING FINANCIAL AID TO PAY FOR THE PROGRAM:
Students using financial aid must complete BOTH forms listed below and return them to ASA by the application/payment deadline. The
form marked "Student" is for you to complete, and the form marked "University" is for your Financial Aid
Officer to complete. Students planning to pay for the program with financial aid must read our
Financial Aid Guidelines.

VI. PROOF OF PHYSICAL EXAMINATION
Proof of Physical form (PDF file) - At least 2 weeks before you depart, you must send ASA this completed
Proof of Physical form, completed by your doctor during a recent physical examination. You
will have to schedule a physical examination with your doctor in order to fulfill this requirement.
VII. PHOTOCOPY OF PASSPORT
If you have not yet submitted a photocopy of your passport to ASA,
please do so immediately. Passport copies must be MAILED to ASA. Faxed passports
come out too blurry to read.
If you don't yet have a passport, apply for one right away! Lately, they have been
taking about 6 weeks to
process by regular service. Don't forget you
will need your passport in hand in order to apply for your Student Visa (if applicable - see below),
and you have to apply
for your student visa 60 days prior to departure, so you may need to have your
passport EXPEDITED in order to get your passport in time to apply for your visa. We
recommend that you have your passport expedited if your departure date is 3 months away or less.
Your passport must not expire for at least 3 months after your proposed return date from abroad. Please check your passport's
expiration date to be sure. Do not plan any international travel during the
months prior to your study abroad program, because you will have to leave your passport
at the Consulate while they process your visa.
VIII. FLIGHT ARRANGEMENTS

Students attending
The Aix Center or the Marchutz School of Art in Aix-en-Provence
for the SUMMER 2010 session should depart from the U.S. on June 11 and arrive at Marseilles airport (MRS)
on June 12 before 5pm. The return date at the end of the Summer 2010 semester is
July 24.
Students attending
The Aix Center or the Marchutz School of Art in Aix-en-Provence
for the FALL 2010 semester should depart from the U.S. on Sept. 3 and arrive at Marseilles airport (MRS)
on Sept. 4 before 5pm. The return date at the end of the Fall 2010 semester is
Dec. 18.
Students attending
Le Centre d'Avignon
for the SUMMER 2010 session should depart from the U.S. on June 15 and arrive at Paris
Charles de Gaulle airport (CDG)
on June 16. You will then take the TGV train
directly from CDG airport to the Avignon TGV station, arriving at the Avignon TGV station between 10am and 6pm.
Your recommended arrival time at CDG is by 10am; please allow 2-3 hours between your flight’s arrival time and your train’s departure time.
You can book your train ticket at www.raileurope.com.
Be sure to book both a 2nd class ticket AND a reservation, and ask for the 12-25 youth fare.
The return date at the end of the Summer 2010 session is
July 28.
Students attending
Le Centre d'Avignon
for the FALL 2010 semester should depart from the U.S. on Sept. 3 and arrive at Paris
Charles de Gaulle airport (CDG)
on Sept. 4. You will then take the TGV train
directly from CDG airport to the Avignon TGV station, arriving at the Avignon TGV station between 10am and 6pm.

IX. STUDENT VISA
If you are studying in France for a period greater than 90 days (i.e. a
semester or academic year), you must obtain a Student Visa
from the Consulate of France prior to departure. Summer students do NOT
need to apply for a visa as long as their program is less than 90 days long.
