Congratulations on being accepted to the ASA Dublin program!

Before you go, there are several forms that we need you to fill out.

YOUR ONLINE & PAPER FORMS ARE DUE BY THE APPLICATION DEADLINE OR BY 2 WEEKS AFTER YOU'RE ACCEPTED, WHICHEVER COMES SOONER. Exceptions to this are 1) Proof of Physical (see below) which simply needs to be turned in before you depart, and 2) Flight Itinerary Form, which is due no later than 4 weeks prior to departure. If you need more time to turn something in, just give us a call to let us know at 888-845-4272. (Please note: If you have applied after the deadline has passed, we'll need your forms even sooner!)

• After you have been accepted, ASA will mail you an Acceptance Packet. It will help clarify the post-acceptance paperwork that we need you to complete. It contains a checklist of forms that are due and information on how to book your flight.

• The Course Pre-registration Form and the Program Contract cannot be completed online, so we'll email them to you, and then mail them to you as part of the Acceptance Packet. These "paper" forms must be completed and faxed or mailed to ASA.

• The majority of the required forms can be filled out quickly and easily online by clicking on the link below. It will only take you a few minutes to fill them out.

• Please use the Forms Checklist that ASA mails to you to keep track of which forms you have submitted!

• ALL students must fax or mail ASA a Proof of Physical Examination from a doctor. This must be turned in before you depart, at the latest. This can be simply a note from your doctor, stating you're in good health to travel and study abroad. ASA doesn't provide a form for this, since most doctors like to use their own form or simply write you a note. You will probably have to schedule a physical exam in order to obtain your Proof of Physical, so plan ahead and make your appointment now!

• Late or missing paperwork may result in diminished selection of courses and housing.

• To complete the online forms, simply click on the link below. You must use Internet Explorer for your browser in order for the forms to work. To access your account, you will need to enter the e-mail address that you put on your application using ALL LOWERCASE.
